EID Instructional Materials Pre-order Form

The form on this page may be used by PSA members to pre-order EID Instructional Materials without sending payment until the desired rental bundles are available for shipping. The form will be submitted directly to the EID Director of Instructional Materials. Notification to submit payment will be sent by E-mail when the pre-ordered rental bundles are available for shipping. Payment must be received prior to shipping. To submit an order or a pre-order via postal mail, download the Instructional Materials Rental Form from the EID Publications web page. Additional information is located on the EID Instructional Materials Service page.
